Modification No. 01 <br /> Page 1 of 2 Pages <br /> Contract Number 09-0I 1-TRN-004 <br /> Contract Title 61h Street Reconstruction Project <br /> Prime Contractor Schwetz Construction, Inc. <br /> PREAMBLE <br /> This modification effects the following changes to the Work: <br /> (1) (Ref. No. 4) Revise the curb radius on I Avenue north of 61h Street to accommodate planned improvements by Skagit <br /> County. <br /> (2) (Ref. No. 5) Add a driveway approach at 1705 61"Street to accommodate new housing construction including a driveway. <br /> (3) (Ref. No. 9) Eliminate the connection of the 6-inch and 12-inch water mains at K Avenue then abandon the 6-inch main. <br /> (4) (Ref. No. 10) Replace and construct the driveway approaches at San Juan Motel which were omitted from the engineering <br /> plans. <br /> (5) (Ref. No. 11) Install four additional ADA compliant sidewalk ramps including roadway excavation and crushed surfacing <br /> based course for subgrade backfill which were omitted from the engineering plans. <br /> (6) (Ref. No. 12) Lease a larger crane to install the Vortech Vault which is part of the Storm Drain Treatment System. <br /> The Reference Numbers(Ref. No.) shown above are cited for administrative purposes only. The Ref. No.'s are generated by the <br /> City's Project Manager for tracking purposes only. <br /> In addition,sales tax is added for Line Items 22 through 26, 51 and 52 which represent work associated with the water main. <br /> Regarding Item (3) above, CITY personnel were planning to abandon the waterline. However, while abandoning the fire hydrant, <br /> a connection between the 12-inch and 6-inch water mains was discovered. Consequently,this work is being assigned to the <br /> CONTRACTOR under this modification. <br /> The bid schedule and total contract price under ARTICLE IV CONTRACT PRICE are modified to reflect these changes. The <br /> contract price is revised from$426,187.12 by$25,110.23 to$451,297.35 including$1,995.23 sales tax. <br /> The following Articles are hereby modified: <br /> ARTICLE I. WORK. <br /> Add a second paragraph that reads: <br /> "The WORK is modified as follows: <br /> (1) REVISE: Revise the curb radius on I Avenue north of 61h Street to accommodate planned improvements by <br /> Skagit County. By revising the curb radius to 25'versus 35',the curb will align with sidewalk and gutter <br /> improvements planned by Skagit County on I Avenue See Exhibit C. <br /> (2) ADD: Construct a driveway approach at 1705 61h Street to accommodate new housing construction <br /> including driveway access off 6' Street. See Exhibit D. <br /> (3) ADD: Remove and replace two cement concrete driveways at the San Juan Hotel. These driveways were <br /> inadvertently omitted from the Engineer Plans. See Exhibit E. <br /> (4) ADD: At 611'Street and I Avenue, abandon the 6-inch waterline by removing the gate valve and reducer <br /> then placing a 6-inch cap with a thrust blocker. At 6th Street and K Avenue, connect to an existing 8-inch <br /> valve and remove the 8-inch cross. Connect to the existing line with a coupler. See Exhibit F." <br /> (5) ADD: Install four additional ADA compliant sidewalk ramps including roadway excavation and crushed surfacing <br /> based course for subgrade backfill. Two ramps at N Avenue will allow for north-south pedestrian travel. At M <br /> Avenue, add two ramps on the east and west sides to allow for proper pedestrian direction. And, remove the <br /> sidewalk on the northeast corner. See Exhibit G. <br /> (6) REVISE: Lease a crane suitable to install a pick up to 37,500 lbs which is the estimated weight of the water quality <br /> vault that was designed as part of this System. The estimated weight quoted by the City during the bid process was <br /> understated. See Exhibit H. <br />
