COMMUnity Paramedic Agreement
'This Community Paramedic Agreement ("Agreernent") is by and between the City of
Anacortes (the "City"), a Washington municipal corporation, and Skagit County Public
Hospital District No. 2 (the "District"), d/b/a Island Hospital, a Washington Municipal
corporation, individually referred to as "Party" or collectively as "Parties".

WHEREAS, Chapter 39.34 RCW regarding interlocal cooperation authorizes
local governmental agencies to make more efficient use of their powers by enabling
them to cooperate with each other on the basis of mutual advantage and thereby to
provide services and facilities in a manner that best accords with geographic,
economic, population, or other factors influencing the needs and development of
each governmental entity; and
WHEREAS, the City of Anacortes and Island Hospital recognize that a
Community Paramedic Program (the "Program") would benefit Island Hospital, the
City, and the community; and,
WHEREAS, the Parties have mutually determined that it would be beneficial to
partner on the Program, by which the City would assign one (1) full-time, experienced,
fully-qualified and certified paramedic as a Community Paramedic to Island Hospital to
assist with its mission to deliver quality, compassionate, and personalized health care
to the community and align with its vision to develop innovative programs that enhance
patient experiences and outcomes; and
WHEREAS, the Parties wish to commit to the partial funding of the paramedic as
described herein;

WHEREAS, it is the intention of the Parties as reflected by the partial funding of
the paramedic to acknowledge that the paramedic, as all paramedics of the City, are
subject to the control of the Chief of the Anacortes Fire Department (hereinafter
"Chief") and may be assigned other duties outside of the normal Community
Paramedic schedule for training and emergencies.
NOW, THEREFORE, the City and the District agree as follows for good and
valuable consideration, the sufficiency of which is hereby acknowledged, and in
accordance with Chapter 39.34 RCW.
AGREEMENT
1. Purpose. The purpose of this agreement is to provide for partial funding
necessary to implement the Program to permit assignment by the City of one
full-time, experienced paramedic in conjunction with the District. The program
will:
a. Provide for budget certainty for the future of the Program.
b. Reduce 911 calls -for high utilizer patients.
c. Reduce hospital readmission rates.
d. Evaluate and identify -future Program goals and rnetrics.
2. Term and Termination
a. This agreement shall commence January 1, 2022, and shall run for one
calendar year, unless terminated earlier. The agreement will automatically
renew -for an unlimited number of subsequent one-year terms, unless
-terminated by either Party by providing 30 days' notice prior to the end of
the then current term. The Parties acknowledge that continuation of the
program requires budget approval and the Parties agree to work
collaboratively to commit to funding within each Party's respective budget

timelines.
b. Either Party may terminate this agreement at any time for any reason
upon 30 days written notice.
3. Community Paramedic Responsibilities. Community Paramedic duties and
responsibilities include but are not limited to the following:
a. Assisting the District in providing high-quality personalized health care.
b. Responding to emergencies or other types of calls.
c. Providing support for patient transports.
d. Providing education and training.
4. City Responsibilities. The City is responsible for the following:
a. Provide a paramedic to be assigned to support the District. Overtime must
be pre-approved by an Anacortes Fire Department supervisor.
b. The City shall provide adequate record keeping and reporting, as required by
federal, state, and local laws, and as necessary to justify invoices for
services under this Agreement.
5. District Responsibilities. The District is responsible for the following:
a. Provide regular feedback and an annual performance review of the
Community Paramedic to the Chief.
b. Provide in-service training as applicable.
c. Provide access to database and recordkeeping programs as necessary for
the administration of this Agreement.
6. Fundinq.
a. The District will pay 50% of the paramedic's applicable salary, benefits, and
premium expenses, including overtime costs. The City will invoice the District
quarterly for the actual costs.

b. The City will pay all remaining costs associated with the paramedic's
employment.
7. Prohibition against assignment. Neither this agreement nor any interest herein
may be assigned by either Party without first obtaining written consent of the
other Party.
8. Independent Contractor Status of City. The City, its employees, and agents
shall perform all duties pursuant to this agreement as an independent
contractor. The District shall not control or supervise the mariner in which this
agreement is performed nor withhold or pay any taxes on behalf of the City of
Anacortes, its employees or agents. The City provides liability coverage for all
City employees under the Washington Cities Insurance Authority municipal
risk pool.
9. Indemnification,
a. Any and all claims which hereafter arise on the part of any and all persons
as a direct or indirect result of the City employee's performance or failure to
perform duties pursuant to this agreement shall be the City's obligation, and
the City shall defend, pay costs of defense, indemnify and hold harmless
the District and the District's employees and agents in -full for any and all
such acts or failures to act on the part of the City or the City's employees or
agents.
b. Any and all claims which hereafter arise on the part of any and all persons
as a direct or indirect result of the District employee's performance or failure
to perform duties pursuant to this agreement shall be the District's sole
obligation, and the District shall defend, pay costs of defense, indemnify and
hold harmless the City and the City's employees and agents in full for any
and all such acts or failures to act on the part of the District or the District's
employees of agents.

11. Applicable Law.
a. This agreement shall be governed by the laws of the State of Washington.
Venue for any legal action shall be proper only in Skagit County Washington.
b. The Parties shall comply, where applicable, with the contract work hours
and Safety Standards Act and collective bargaining agreements, and any
other federal and state statutes, rules, and regulations,

c. Organization of separate entity and its powers - no new or separate legal
or administrative entity is created to administer the provisions of this
agreement.
d. Responsibilities of parties - see section 4 and 5.

Executive Summary
Community Paramedicine Program
Statement of Problem,
Island Health Emergency Department experiences visit from high utilizer patients that many times do not require ED
level of care. Currently we do not have an effective way to intercept and redirect these patients to more appropriate
care which would both reduce costs and improve health outcomes.
Recommended Solution:
A community partnership with Anacortes Fire Department (AFD) in a Community Paramedic Program creates a mobile
integrated healthcare program. In this program AFD also partners with:
• Anacortes Family Center • Health Common Project
• Anacortes Police Department • Fidalgo Rotary
• NW Regional Council • Anacortes Firefighters Union
• NSACH
This program employs an AFD Paramedic who works in collaboration with Island Health and the other community
partners to complete home visits with identified patients to assess, report, refer patients to appropriate health, social,
and community resources.
Proiect Goals:
1. Decrease healthcare cost by reducing unnecessary ED visits by 20 visits July—December 2022. Visits will be
mI tracked against enrollees previous visit rate.
2. Improve health of IH patients through identification of patients to enroll in Chronic Care and Transitional care
programs. Goal to enroll }50%of eligible patients in the CPP with AFD.
3. Reduce risk and improve quality of life in the community with home visits, homeless outreach, and linkage to
medical care, social services, and community resources. Measured quarterly by enrollees:
• Visits by date • Entry point
• Status • Calls tracked prior and post enrolling
• Problem Category
Proiect Obiectives:
1. Reduce burden on IH ED and AFD from 911 calls for non-emergent visits by frequent utilizers
2. Reduce burden on IH from unnecessary and inappropriate ED visits and prevent readmissions post
hospitalization
3. Refer underserved and vulnerable patients to appropriate healthcare services and social and community
resources.

This program will utilize patient home visits and homeless outreach to achieve stated program goals and objectives. The
Community Paramedic will work collaborative with the IH ED and Primary Care Clinic teams to identify patients,to
assess and report on these patients enabling us to enroll patients in our CCM and TCM programs. This will not only
reduce the cost of patients care but will allow us the opportunity to have an impact on improving their health status.
The community collaborative program will improve the health and safety of our Community.
