Currently we do not have an effective way to intercept and redirect these patients to more appropriate care which would both reduce costs and improve health outcomes. In this program AFD also partners with: • Anacortes Family Center • Health Common Project • Anacortes Police Department • Fidalgo Rotary • NW Regional Council • Anacortes Firefighters Union • NSACH This program employs an AFD Paramedic who works in collaboration with Island Health and the other community partners to complete home visits with identified patients to assess, report, refer patients to appropriate health, social, and community resources. Proiect Goals: 1. Decrease healthcare cost by reducing unnecessary ED visits by 20 visits July—December 2022. Visits will be mI tracked against enrollees previous visit rate. 2. Improve health of IH patients through identification of patients to enroll in Chronic Care and Transitional care programs. Goal to enroll }50%of eligible patients in the CPP with AFD. 3. Reduce risk and improve quality of life in the community with home visits, homeless outreach, and linkage to medical care, social services, and community resources.
