Remarks:
MTC was on site as requested to observe the placement of 14+ yards of mix design 0155P delivery by Skagit ready mix
into the basement slab on grade noted above. Reinforcing was previously inspected on 5/7/2021, refer to report
F255353 for details.. Concrete was place via boom pump and consolidated and finished by hand. While onsite MTC
collected a composit sample of concrete and made one set of five 4x8 cylinders to test for 4000 psi compressive stregth
requirement.
To the best of MTC inspector's knowledge, the above -described work was performed in general accordance with project
specifications and approved plans.
