MTC inspector on site, as requested, to observe the placement of approximately 60 CY of concrete with Mix Design ID
#01650 supplied by Skagit Ready Mix into walls, at the location noted above.
One set of five (4" x 8") cylinders was cast for compressive strength test specimens. Refer to Report #C254216 for field
test results and for more information.
Concrete was placed via concrete pump, was mechanically consolidated with high -cycle vibrator, and finished by hand.
The technician was also on -site to perform a hold-down inspection. All hold downs were verified, accounted for and
placed in the correct locations. However, the anchors were placed outside the 2 horizontal bars as per the directions of
the EOR.
To the best of MTC inspector's knowledge, the above -described work was performed in general accordance with project
specifications and approved plans.
