MTC on site as requested to visually inspect reinforcing steel at the following location(s):
1.) MTC observed the work in progress of the reinforcing steel for the building foundation grade beams and footings at
grid lines (A-D/1-6). MTC observed that the footings were inspected per plan sheet S2.1 and the typical details called out.
MTC observed that the wood shear wall hold down anchor bolts were being installed per plan layout sheet S2.1. See
below for exceptions taken.
-Reinforcing Steel Inspection:
1.) MTC visually inspected the reinforcing steel at the locations noted and for the exception of the items noted above,
observed that the reinforcing steel complied with the approved project plans and specifications. MTC observed the
reinforcing steel size, grade, type, and placement. MTC will verify the final clearances prior to and during concrete
placement.
Exceptions noted:
1. MTC notes that the grade beam typical detail on sheet S3.1 details #5 transverse bars top and
bottom at 12" on center. MTC observed that the grade beams were reinforced with #4 beam ties at
12" on center.
2. MTC notes that the typical details specify additional (2) #4 beam ties 4" on center at every hold
down anchor rod. MTC observed that these additional ties were not installed.
3. MTC observed that the typical thickened footing detail 10 on sheet S3.1 details a thickened footing
with (7) #5 longitudinal bars top and bottom. MTC notes that these thickened footings were built
as low footings with (5) #4 longitudinal bars and a stem wall to extend up above the slab on grade.
4. MTC observed that the HDU8 7/8" hold down anchor bolts were not installed per sheet S2.1 and 4
on sheet S4.1. These anchor rods need to be embedded 18" into the concrete. There are (6) total
HDU8 per plan.
5. Contractor to ensure proper clearances are provided for all reinforcing steel. MTC observed that
reinforcing steel was burning on conduit penetrations at multiple locations.

MTC was notified by the on site general contractor representative that there had been approved changes to the permit
set of drawings but did not have copies of the changes at this time. The general contractor will follow up with MTC to
provide any approved changes and a re -inspection prior to concrete placement.
Work in progress. Additional inspection needed.
