I agree to the scope of work and contract price as quoted, plus sales tax and permit if required. I further agree to pay in full the contract amount
upon completion of work, unless otherwise stated in writing. I have read the terms and conditions of this contract and acknowledge receipt of
my copy. I agree that all parts and equipment will be discarded unless otherwise specified. I hereby agree to the contract and authorize work to
be performed. Cpi will not warranty drain cleaning. NOTE: All drain cleaning is an attempt only to clean drains

1 have authority to order the work outlined above which has been satisfactorily completed. I agree that seller retains title to
equipment/materials furnished until final payment is made. If payment is not made as agreed, seller can remove said equipment at sellers
expense. Any damage resulting from said removal shall be the responsibility of seller. See all terms, warranty and conditions.

Terms: Balance is due in full, at time of service unless otherwise arranged. A service charge 1.5% per
month (18% annum) will be applied to all past due accounts.$1.50 minimum. If this matter is placed in
the hands of an attorney for collection without regard to whether suit is instituted, purchaser herby
agrees to pay reasonable fees for the cost of collection of this account. Because it is impossible to
estimate collection costs including attorney fees, the undersigned hereby promises to pay a minimum
$500.00 attorney fees together with the costs of collection.
Kelly's Club Memberships: If a membership is cancelled within the 12 month enrollment period the
customer is responsible for the difference between the Club Member price and the Standard price
(Typically 1S%).
Annual memberships expire one year from initial purchase date.
Monthly memberships will auto -renew on the first day of your renewal month.
