Zevely 5112 Castle Court Anacortes, Washington 98221 Reference: 92-1-00467 Zevely, James Dear Mr. Zevely: You are hereby authorized to construct a floating dock, install a ramp, and dredge up to 6 cubic yards in Flounder Bay at Anacortes, Washington. The work must be performed as depicted on the enclosed drawing (enclosure 1), and you must meet the enclosed conditions (enclosure 2) . Please pay particular attention to Special Conditions a through e. We have reviewed your application for a Department of the Army permit for this work. Our review was made pursuant to Section 10 of the Rivers and Harbors Act of March 3, 1899. We determined that your proposal is minor in nature, will not significantly impact environmental values, and should encounter no opposition. We are thus issuing you a "Letter of Permission" to authorize the work, rather than using our lengthier public notice procedures. Please note the conditions and limitations contained in enclosure 2. Read them carefully. You should also be aware that any changes in the location or plans for this work will require submittal of a revised drawing to this office, and our written approval, prior to accomplishment. If piling are to be used for your project, we recommend the use of non-creosote piling. While you need no further authorization from us, you must still comply with other local and State requirements. At a minimum, you must obtain a Shorelines Substantial Development Permit determination from the city of Anacortes, and a Hydraulic Project Approval from the Washington State Department of Fisheries and Department of Wildlife, and you must contact the Washington State Department of Natural Resources for a determination of leasing requirements for submerged lands before you begin your work. Requirements are grouped by foundation, framing, insulation, and final inspection phases. This not only lets you know what you need to do but also when the inspector will be checking for particular requirements. Use the form to choose compliance options that best suit the economics and design of your project. A designated department energy specialist can help you with your questions. This form will help you comply with the WSEC as quickly and painlessly as possible. Responsibility for information: Although staff members will help you with general questions about © completing •this •form, it•is •ultimately your responsibility •to provide •detailed •information about heating systems, glazing, Insulation, and other building specifications.Be sure to get prior approval from the Department if you later wish to make changes in your project. For more requirement detail, refer to the Washington State ,i_.... Energy Code, 1991 Edition. Page 1, Compliance Options: Select one compliance option at the top of the table. Pages 2 through 6, Jn the shaded box areas only; Provide information and check circles that Oapply to your specific project. Do not place marks in the two left side column(labeled"COMPLIANCE REQUIRED" and "INSPECTION APPROVED" or in boxes located in non-shaded areas (These are for ' department use). Since this form will be evaluated for completeness and accuracy, you can avoid unecessary permit delays ' by carefully providing all required information. You may disregard items that don't address your particular building or equipment. .- CICompliance responsibility:This form will ultimately be used for all WSEC field inspections.
