GPRequest for a Proposal to furnish all materials and labor as required to fulfill thf I L E C following scope of work: PROTECTION: Prior to start of demolition, the contractor is required to tent the work areas with plastic and to provide negative pressure ventilation 24/7 during all operations that will cause dust migration. Any pathways used from the exterior doors to the work area require protective measures such as ram board or plywood to ensure the existing floors are not damaged and are kept clean. CONCRETE DEMOLITION & REPLACEMENT: Sawcut and remove 8" concrete slab as detailed on structural drawings S1.1 dated 3-29-2019 and supporting calculations dated 3-29-2019. Note that it is critical to follow ALL CRITERIA NOTES shown on the structural plan, including limitations on cutting rebar. Note that scanning is required whether with X-ray or ground penetrating radar, whichever is more suitable to accurately locate reinforcing. Concrete and rebar pin and reinforcing requirements are called out on the structural drawing. Contractor is required to schedule building department inspections and special inspections and test samples with the owner's selected special inspector prior to any concrete placement. Costs for special inspector will be paid by the owner. Hours for noisy demolition will be limited due to office workers located above the work area and because the building has an open air plan. Noise limitations apply from 7:30am to 4:30pm M— F. PLUMBING REPLACEMENT: It is expected that a vacuum truck will be required to remove gravel from the existing plumbing trench. See attached sketch of the existing sewer pipe location. Do not rely on this sketch. Contractor is required to locate existing sewer pipe prior to the start of demolition. The new replacement sewer pipe is required from the connection of the first fixture all the way to the exterior manhole. Replace all existing 4"ABS pipe with 4" schedule 40 PVC pressure pipe (Not foam core) and connect to all existing branch drains and vents. New piping to be hung from slab at 4' intervals with stainless steel clevis hangers, threaded rod and hardware per the attached detail. Bed and backfill above new pipe with pea gravel. Camera inspect all remaining existing connections and piping including vertical piping prior to new connections. SCHEDULE: Time is of the essence for this project. Contractor to start as soon as possible after bid award and to work continuously through completion. CLEAN UP: Contractor is responsible for all construction related cleanup during construction and final cleaning of work areas after completion. OTHER REQUIREMENTS: - This is a Public Work that is subject to prevailing wages and other public works statutes. Contractor/subcontractors required to file Statement of Intent to Pay Prevailing Wages immediately upon contract award and Affidavit of Wages Paid upon completion of project. Contractor and subcontractors required to provide proof of insurance coverage via endorsements naming the NWESD as additional insured on those policies for all projects, regardless of dollar size. Upon completion of project, NWESD will file a Notice of Completion with Department of Labor& Industries, Department of Revenue, and Employment Security Department and withhold 5% of the contract amount until all three agencies separately approve release of the retainage. - Building Permit: Contractor to assist owner in obtaining the building permit. The building permit fee will be paid by the owner. City business license to be paid by contractor. - Include in your quote the cost for a performance and payment bond. The site has very poor soil conditions and as a result, the original design of the foundation required the use of piles and a structural slab. The following pages focus on strength checks of the existing slab in the temporary condition where small sections of the slab will need to be demolished and then replaced. The purpose of localized demolition of the slab is to access below grade pipes that have been damaged due to the poor soil conditions. Many pipes have deformed as they sag with the sinking soil and need to be repaired or replaced. The only way to access these pipes is to cut openings in localized areas of the slab.
