Dear Don,
We are submitting for permit on behalf of the North Cascade Ear,Nose,&Throat Clinic for tenant
improvements at an existing location. The address for the proposed construction is 1019 24m Street,
Suite B in Anacortes. This building is the current location for Bill Bethards D.D.S. dental practice,which
is located in the adjacent suite.The building's original construction date was somewhere around 1990.
I have included a scaled existing floor plan,plan drawings for the new tenant improvement,and an HVAC
layout for the new rooms.
• Wall Construction: 2x4 wood frame walls or 25 ga.metal stud with sound batt insulation,5/8" GWB
on all walls
• Ceilings: The existing space has suspended acoustic grid ceilings with recessed light troffers
Some ceiling areas will be removed to accommodate new rooms. The new finished ceiling will be
similar.
• Plumbing: New sink areas as per plan. An accessible bathroom is presently located in the building.A
new bathroom will also be added at the front reception area as indicated,with sheet vinyl floors and p-
lain wainscot to 48"in height. Both bathrooms will be unisex. A backflow prevention device will be
added to the water supply in the crawl space.
• Mechanical: The existing fumace/heat pump combination that is currently provided to the space will
be retained. The plan calls for a rezoning of the space into 3 zones,a supply in each room and transfer
grills for the air movement.
• Electrical: The existing lighting,troffers,parabolics,etc. will be retained and used in the new
locations. Due to no change in the HVAC there will be no electrical required to those areas.
• Cabinets/Casework: New cabinets and casework per plan. The reception desk is existing and will
more than likely be retained.

In response to your letter of May 22'(attached) I have addressed each of your concerns
individually:
1. Copy of current Washington State Contractor's License aaaehed r
2. We would prefer to reduce the occupant load by considering the waiting area as an accessory
use of the
Office space. If that is not fc asiblo we can delete the accordion door from the improvements.
3. Signage directing clients to the barrier free restroom will be installed on the hallway wall to
the left side of the scope room pocket door.This location provides easy visibility for clients at
the reception or waiting room,
4. As discussed,we will be installing lever set door hardware at all locations and barrier free
hardware at the pocket doors.
5. We wilt be installing a reduced pressure backflow assy-in the crawl space at the water source
for the tenant space. Commercial Plumbing Inc. will be the installation contractor.
6. I am attaching a cut sheet which shows the actual weight of the manufactured audio booth.
This will be one of the last items to be installed in the space and the architect will be
verifying the existing structure.
7_ The new restroom floor will be commercial sbeet vinyl and the vinyl will continue up the
wall for a minimum of 5"with heat welded seams.
