Review only covers that portion of the construction documents that involves remodeling of an existing
boarding home unit for use as Storage 220 & Corridor 221, and the addition of Connection 219 &
Dining 222.

This project is not approved and is not yet authorized for use by the Licensing Agency.
Provide written responses to our review for those items checked as not approved. To help expedite our
review include a transmittal with all submissions clearly identifying all documents with your project name
and CRS project number.
Include two copies of revised plans or sheets, in the same format/size as the original submission,
incorporating your corrections. Any response that does not address ALL the review comments and provide
the required information will be considered incomplete and may be returned without review.
All documents submitted will be retained in our office until all the construction documents have been
received, reviewed, and accepted. When the project is approved, a complete package of documents will be
stamped and signed by the department. One copy will be returned to the facility administrator and one
retained for our records.

1 ❑x Construction and renovation may create conditions that compromise the health and safety
of residents, staff, and visitors. Facility planning must include, in addition to space and
operational needs, provisions for infection control and safety of the facility's occupants
during any renovation or new construction. WAC 388-78A-2850(3)
Per the Infection Control Procedure information the proposed construction is
i to be separated from existing portions of the facility by existing wall assemblies
and/or 6 mil plastic sheet barriers. The Contractor will be directed to keep the
existing corridor clear of tools, supplies, and debris materials.

incorporated into the Construction documents.

3 0 Per the functional program the independent residents shall be allowed to dining and
activities with the boarding home residents. Provide drawings showing how the
combined facility will provide at least twenty square of common area for the combined
total number of the residents, both boarding home and independent, as if all were
boarding home residents. WAC 388-78A-3050(2)(b)
Architect response received 10106109—Attached please find CRS letter dated
March 12th, 2009 which documents the existing facility is occupied by 78
residents requiring 1,560 square feet of common space. Per the March 12th
floor plans, the existing facility contains 3,000 square feet of common space

excluding Private Dining Room#264. There has been no change to the
existing facility since March 12`h, 2009.
The 25 apartment Independent Living Wing is potentially occupied by a
maximum of 50 residents. The Independent Living Wing contains 350 square
foot Lobby#216 and a 653 square foot Rec. Room#124 (1,003 square feet)of
common space for use by Independent Living Residents.
The new construction will also include a 396 square foot Dining Room
Addition and an 843 square foot Connection/Assembly (1,239 square feet) to be
used in common by both the existing facility and the independent living wing
residents.
The total of occupants is 78 (existing)— 1 (apartment removed) + 50 (new
independent) = 127 residents requiring 2,540 square feet of common space.
The facility contains over 3,000 + 1,239 = 4,239 square feet of common space
to be used by all residents. There is also 1,003 square feet of common space to
be used by the independent living wing residents.
Approved 11/13/09—Per the written response a maximum of 50 occupants will be
added to the complex. Between the Dining Room Addition and the
Connection/Assembly area more than the minimum 20 square feet per resident for
common area will be added.
4 ❑x Provide a drawing locating a means of communication in Connection 219 for residents
and staff to contact on-duty staff in an emergency. WAC 388-78A-2930(1)(a)(ii)
Architect response received 10106109—Shall comply, Shop Drawings of
emergency call system shall be provided by Commercial Alarm &Detection. It
is intended to be a wireless system consisting of pendants for residents, radios
for staff, and wall mounted wireless devices installed at required locations.
Shop Drawings are to be reviewed and approved by CRS prior to installation.
See Deferred Submittals on sheet Al.
Noted 11/13/09—The means of communication documents shall be submitted as a
deferred submittal.
5 0 Provide manufacturer's labeled carpet specifications and radiant panel test report for
each carpet style used in and exiting from the boarding home licensed portions of this
project. WAC 388-78A-2850(1)(b)(vi), and Section 804, International Building Code
Architect response received 10106109—Shall comply, attached please find
Addendum#1. Project Manual Items#6, #7, and#8 specify the carpet and pad
to be uses. Attached please also find radiant panel test reports.
Not Approved 11/13/09—Carpet styles Sable Island and Legend Falls by

Philadelphia Commercial are approved for direct glue-down installation.
However,per Project Manual Items#7 and#8 carpeting is to be installed over pads
and not direct glued as was how the carpets were tested. Provide revised
information removing the pads and directly securing the carpet to the floor,or
provide information showing the carpet with pad testing reports and how the
carpet/pad assembly qualifies as carpet with nonskid backing.
6 ❑x Provide a drawing showing the potential layout of table and chairs in Dining Addition
222 for verification an aisle exit access to exit Door 186 can be maintained.
WAC 388-78A-2910(1), Sec. 1014.4 International Building Code
Architect response received 10106109—Shall comply, attached please find
supplemental drawing SDI dated 09128109. The table%hair spaces indicated
include the table width plus 19" each side for chairs (IBC 1014.4.3). The
distance between the table%hair spaces is required to be a minimum of 0.15"x
4 chairs =0.6" (IBC 1014.4.3.2 exception and TBL 1005). Per IBC 1109.11, a
minimum of 5% of the total Dining Room seating (2 spaces)are accessible per
ICC ANSI A117.1.
Approved 11/13/09—Per SD1 dated 09/28/09 the tables and chairs can be arranged
to provide seating for 26 while maintaining at least a 44"aisle to exit Door 186.
7 0 Provide revised drawings showing/verify the existing exterior of the boarding home on
each side of the 2-hr. fire wall and roof overhang are a one-hour rated assemblies with 3/4
hr. rated openings, or explain why the building code does not require the ratings of the
adjoining surfaces. WAC 388-78A-2910(1), Sec. 705.5.1 International Building Code
Architect response received 10106109—Shall comply, Please see detail F/A16.
The 2 hour fire wall is required to be designed in accordance with IBC Section
705.
1) Per IBC 705.5 Horizontal continuity, Exception 3.: Fire walls shall be
permit to terminate at the interior surface of noncombustible exterior
sheathing where the building on each side of the fire wall is protected by an
automatic sprinkler system installed in accordance with Section 903.3.L I
or 903.3.1.2. According to the construction drawings the existing facility is
constructed using 5/8" type X gypsum sheathing. This can be verified at
the time of demolition. The exterior walls of Connection 219 shall be
revised to be constructed using 5/8" type X gypsum sheathing in lieu of the
OSB sheathing specified. All exterior wall shall have noncombustible
exterior cement board siding.
Not Approved 11/13/09—Per the written response the 2-hour fire wall may
terminate at the interior surface of noncombustible exterior sheathing. This
response does not address the comment requiring the intersected walls to be fire
rated which is a separate issue.

8 ❑x Provide revised drawings showing/verify the existing exterior of the boarding home
vertically above the proposed 2-hr. fire wall is a one-hour rated assembly with 3/4 hr. rated
openings, or explain why the building code does not require the ratings of the above new
roof surfaces. WAC 388-78A-2910(1), Sec. 705.6.1 International Building Code
Architect response received 10106109—Shall comply, Please see detail F/A16.
The 2 hour fire wall is required to be designed in accordance with IBC Section
705. Per IBC 705.6.1 Stepped Buildings, Exception: Where the fire wall
terminates at the underside of the roof sheathing, deck or slab of the lower roof
provided:
2) The lower roof assembly within 10 feet of the wall has not less than a 1-
hour fire-resistance-rating and the entire length and span of supporting
elements for the rated roof assembly has afire-resistance rating of not less
than I hour.
3) Openings in the lower roof shall not be located within 10 feet of the fire
wall.
The I hour ceiling construction shall be installed per GA file no. RC 2601
The 2 hour wall construction shall be installed per UL Design No. U301

Approved 11/13/09—Per the written response the roof construction will be a one-
hour fire resistance rated assembly. It is assumed no openings will be located
within 10 feet of the fire wall. It is also assumed the City of Anacortes approves this
design even though a copy of the review response letter was not located.
9 ❑x Provide revised Sheet Al showing the two new patios accessed from Doors 05 &06 and
the new sidewalk accessed from Door 186 connected to the public way with direct and
unobstructed access. WAC 388-78A-2910(1), Sec. 1024.6 International Building Code
Architect response received 10106109—Shall comply, attached please find
revised drawing AI dated 09128109. In addition we will be submitting a
Landscape Plan as a Deferred Submittal which will clearly identify the
sidewalks and patios.
Approved 11/13/09—Revised Sheet Al shows the two new patios connecting to the
public way with direct and unobstructed access walkways.
10 x❑ Constructing Corridor 221 will create a dead end in excess of twenty feet. Provide
revised drawings, or explain why this portion of the corridor system should not be
considered a dead end.
WAC 388-78A-2910(1), Sec. 1017.3 International Building Code
Architect response received 10106109—Shall comply, attached please find
supplemental drawing SD2 and AD2A dated 09128109. The maximum corridor

depth is 24'. Per IBC 1017.3 Dead Ends, Exception 3, the minimum with is
24'/2.5 = 9'-7 3/16" in order to avoid being a dead end corridor. The existing
wall above door#58 has been removed to create a minimum with of 9'-11".
Approved 11/13/09—Per drawing SD2A the minimum width will be 9'-11"allowing
a deadend distance up to 24'-91h" which is greater than the 24'-7"or so that will
occur when the new 2-hour wall is installed.
11 x❑ Provide two complete plans and specifications for the fire alarm system installation or
modification for review and approval prior to system installation. Plans and
specifications shall include, but not be limited to, a floor plan; location of all alarm-
initiating and alarm-signaling devices; alarm-control and trouble-signaling equipment;
annunciation; power connection; battery calculations; conductor type and sizes; voltage
drop calculations; name, address, and phone number of the agency receiving off-
premises transmission of alarm; and the manufacturer, model numbers, and listing
information for all equipment, devices, and materials. Incomplete Plans and
specifications will be returned without review. Plans and specifications may be
submitted separately from construction documents during the construction of the project.
The department reserves the right to defer plan review and inspections to the local
authority having jurisdiction (AHJ). Section 907.1, International Fire Code
Architect response received 10106109—Shall comply, Fire Alarm System
documentation shall be provided as a Deferred Submittal for CRS review and
approval prior to installation.
Noted 11/13/09—Fire alarm documents to be a deferred submittal.
12 0 Provide two sets of sprinkler system working plans for review and approval before any
equipment is installed or remodeled. Deviation from approved plans will require
permission. Plans and specifications including ydraulic calculations that are
incomplete or are not stamped by a Washington State Licensed Fire Sprinkler Contractor,
will be returned without review. Plans and specifications may be submitted separately
from construction documents during the construction of the project. The department
reserves the right to defer plan review and inspections to the local authority having
jurisdiction (AHJ). Section 903.1, International Fire Code
Architect response received 10106109—Shall comply, Sprinkler System
documentation shall be provided as a Deferred Submittal for CRS review and
approval prior to installation.
Noted 11/13/09—Fire sprinkler documents to be a deferred submittal.
Compliance with the comments above provided by the Department of Health, Construction Review Services,are necessary for this facility to meet
the requirements of the applicable licensing regulations found in the Washington State Administrative Code and associated references. These
comments do not relieve the facility from the responsibility to meet the requirements of any other applicable federal, state or local regulations.
In the event of conflicts between other jurisdictions and these written comments, the most stringent shall apply.
