EAS (contractor #ENVIRAS014RA) removed asbestos containing material from the above _address as specified in the survey. According to local state and federal regulatictos, materials containing more than one percent (1 %) asbestos must be removed prior to any remodel, renovation, or demolition activity. The material was removed by Certified Asbestos Workers under the direct supervision of a Certified Asbestos Supervisor in accordance with all applicable local, state and federal regulations on 02/19/2025. The materials collected were packaged for final disposal in compliance with all applicable federal, state, and local laws. If materic1ls are identified under 1 % asbestos on the survey work practices and worker protection must be in compliance with WAC 2962-62-077. Sincerely, Expires 7/31/2025 1 Paul Palu On the above date I performed an inspection of your jobsite, as described in the workscope, for asbestos containing materials. I took samples from visible and accessible materials including walls, floors, and ceilings to test for the presence of asbestos. The samples were analyzed at an NVLAP accredited lab using Polarized Light Microscopy, see attached. Building materials are regulated under Title 40 Code of Federal Regulations (40 CFR), subpart M, section 61.141. This regulation sets the allowable limit of asbestos in building materials at 1%. Materials containing more than 1% asbestos must be handled in accordance with Federal, State, and Local regulations. Although reasonable effort was made to locate and test all suspect materials, some materials may be hidden beneath floors or other structures until demolition or renovation makes them accessible. If other materials are discovered during the course of a demolition or remodel, they must be tested prior to continuing work. Any materials not listed on this report must be presumed to contain asbestos.
