Below are the following items that need immediate attention. <br /> 1. Install an asphalt patch over the waterline trench across Georgia Avenue at <br /> Oakes Avenue. <br /> 2. Install DOE approved erosion control BMP's along Georgia Avenue. Existing <br /> signs of erosion between the should of the road and the property line. <br /> 3. Stabilize all exposed areas with DOE approved BMP's. <br /> 4. Re-establish the inlet protection at the Storm Drain Manhole. Current method of <br /> installation is showing to be ineffective. <br /> 5. Provide a Large Parcel Storm Water Plan and Erosion Control Plan for review <br /> and approval by the City of Anacortes. This report is required because of the <br /> 5,000 SF of disturbed soils due to construction activities. <br /> 6. Remove all bedding material and construction debris from the Public Right-of- <br /> Way along Georgia and Oakes Avenue. <br /> 7. Re-establish the construction fencing along Oakes Avenue. <br /> 8. Re-establish the road shoulder along Oakes Avenue from Georgia Avenue to <br /> Florida Avenue. <br /> 9. Re-establish the water quality swale along the south side of Oakes Avenue from <br /> Florida Avenue towards Georgia Avenue. <br /> 10. Provide all of the weekly Erosion Control Reports. <br /> 11. Provide a copy of the Erosion Control Leads Certificate to the City of Anacortes <br /> Engineering Department. <br />
